Inconsistent visibility of items and mysteries on Pathfinder exploration level

I played at Pathfinder, and it worked well for most of the game, but the visibility is very inconsistent. Some items and discoveries are visible across half the map, while others are not marked even if you walk right next to them and scan the whole area with your raven.
When I had most of them done, and had scanned the whole of England over and over for many many hours without finding anything at all, I gave up and changed exploration to default, at which point all of the remaining ones popped up.
