Closing the game and restarting worked for me to make him spawn. I am playing on Series X if that makes any difference.

I am also having this problem. It is stuck on “Follow Aelfgar”. He is not moving. I’ve tried fast traveling and coming back, closing and restarting the game, and loading an earlier save to have it happen again.

@jh081991 I got this achievement last night in Werham (Hamtunscire). It took me around 10-15 tries, I think. A few things that seemed to make it work:

1. Stand in the fire until your meter builds all the way up. Being on fire may not count unless it builds up all the way.
2. Make sure you do not drop combat between the kills.
3. make sure you get the killing blow on the guards. I think fire/burning was responsible for killing at least one of them several times I tried.I finally did it by killing with a combat of bow and melee attacks.
