@Ubi-Thrupney I have an update that I believe may be the root cause. I was reading the other forum post about the game not starting at all after the 1.2.1 update. A poster said that he set Ubisoft connect app to OFFLINE mode and he was able to get the game to load. I tried this and my game no longer crashes on the loading screens. Also i notice that the ugly Vaas face loading screens are now gone (thank you!!!) and have been replaced with the original loading screens with the region name on them. I hope this really helps all of the other people having this issue.
@Ubi-Thrupney Below is my findings from this issue. This is from a support ticket I opened with ubisoft about this issue.
Re-installing ubisoft connect did not fix it. Verifying game files did not find any errors and did not fix it. Also I have more specific info on when it will happen. I will define "loading screen", a loading screen is a screen where there is a little yellow icon in the lower right corner of the screen. It only happens on the loading screen with a picture. It only happens after I load into the game for the first time. I can load into the game and see the ugly Vaas face screen during the initial load and the game loads fine. It will crash if I die, fast travel, or attempt to load a special operation. For the special operation it does not show Vaas' face but a picture pertaining to the particular special operation . It does not crash on the black loading screens with the yellow icon that appear when you run out of time on a gran premio race or on the black loading screens with the yellow icon that appear when you run the video benchmark. It only crashes on loading screens with pictures. Because of this I can no longer complete insurgencies as they require a special operation to be completed. I have not tested any multiplayer loading screens because I do not have any friends to test it with. I might be able to test multiplayer tonight with a friend who just got the game. The game will also crash if you quit to main menu and then attempt to load the game again. I do not own the new DLC.
@jason4078 You said it perfectly. I also have a open ticket on this issue. It would be nice to get any sort of acknowledgement from Ubisoft about this issue. This is what these forums are for. Communication to and from the community to solve problems. All I hear are crickets from Ubisoft.
An update to my above statement. I did find that this forum post is now listed in the "Reported Issue Megathread List | Check here for your issue! [UPDATED 15/11/2021]" So that is at least good to know that they are looking at the issue. Would still be nice to hear from them in this post.
@angry_bavarian Yes, none of the special operations can be done in offline mode. I noticed that the Modena I did have was now reported as 0(zero) in my inventory while in offline mode, however it reappeared after switching back to online mode. All of my outfits and weapons purchased through ubisoftconnect remained in my arsenal however purchased vehicles were not available in offline mode. The did reappear once back in online mode. Annoying but not really that important. It may be possible to do all but the special operations part of the insurgency but that test will have to wait until next Monday.
I noticed this morning that while loading into the game the ugly Vaas face loading screen is gone, replaced by the old region-specific loading screens, however the crash issue still persists. Got my hopes up but it is still broken. The lack of communication from the support forum staff is perceived by the players as a lack of caring and that we are being ignored. Please respond with anything regarding the status of this issue.
So as of this morning when I logged in, I looked at the insurgency and saw all of the objectives. This afternoon I log in and the all of the anti-aircraft guns are missing from the map along with some of the other FND bases and checkpoints. I was able to get 2 checkpoints and 1 FND base but I can no longer destroy the AA guns. Appears they have not resolved this issue.
Just some more info to put in this thread. Maybe it will help someone.
I have experienced 2 more times when the game crashes to desktop. First, if I load my single player game then hit ESC and go to the store. Then I browse some items in the store and then hit ESC to go back to my game It will CtD the moment I hit ESC to get out of the store. If I access the store directly from the main menu before loading my save game and I ESC the store the game DOES NOT CtD, it returns me to the main menu. Second, when I access the Insanity DLC from the TV at a gurilla hideout and then choose "Leave and return to Yara" option the game will CtD. The game DOES NOT CtD when accessing and then leaving the Insanity DLC from the "ADD ONS" button on the main menu.
The new loadouts are great. Please consider adding 4 more loadout slots. That would be wonderful.
I can happily report that the Dec 15th hotfix has resolved this issue for me.

@ubi-gizmo Thank you for posting this. Really appreciate it. Please see my recent update I posted about a few minutes ago. More feedback to take to the team. Hopefully it will help.
Well there appeared to be some change to the ubisoft connect client overnight because when I started FC6 this morning I was prompted 3 times to allow ubisoft connect to make changes to my computer. Unfortunately, the crash issue still persists.

@snowyrichmani Yes they have been looking at this one for a few weeks now. I opened a ticket for this issue a while back and their response to me was "they are aware and working on it. No time frame because the devs dont communicate back to the support forum staff. The patch will drop when the patch drops." As of right now unfortunately the only solution is to start a new game and avoid doing the "help the farmers" yaran story in El Este. It does suck but there isn't anything else we can do. I was 28% when this bug bit me and I started over. Not to big of a deal for me because I was able to play the game a bit more efficiently since I had learned from my first playthrough.
