Just wanted to give my 2 cents. I have been having these crashes since druids came out.
What always worked for me is to take Ubisoft offline (doubt other platforms except PC can actually do this). After doing so I can play without crash.
To take it offline click on the 3 horizontal dots top left and select Go Offline.
After this just play the game.

Like many other I have also been experiencing issue.
At first this was resolved by playing offline. However, after 1.4.1.1 even that started to crash.
Tried reinstalling game, GPU driver, moving install to same drive as Ubisoft launcher and nothing seemed to help.
What finally have been working for me for the past 6 days (played a couple of hours every night – only experienced one crash in Ravensthorpe).
After adjusting the fan curve in BIOS and using MSI after burner to adjust the GPU the following have been changed
So, in my case (even though it was still using stock settings) lowering the voltage and upping the cooling did the trick for me – along with verifying game files 3 times (again, seriously?!)
If others are interesting MSI Afterburner is free, and easily to setup, saw a tutorial on YouTube that lasted 5 minutes and I was good to go.
For monitoring temperatures, and fan speeds of GPU I used HWMONITOR from CPUID.
Just reached Jorvik.
Crashed within 15 seconds after entering city- have not been able to keep the game going for more than 2 minutes then it is CTD.
Now I can keep the game alive by using quick travel, but remaining in once area for two minutes and it is CTD.
Absolutely lovely- great job on the new patch
Sadly, the truth is that they are going to do absolutely nothing about it. I have not even seen a post anywhere in which they acknowledge the problem a side from the usual 10 steps of repair and send in a ton of file before you are even worthy a second post.
This issue already started a year ago -
seemingly they have a lot of time to develop new content which just makes it
increasingly worse over time.

For me personally the crashing started with the introduction of last years Yule event. Since then, it has only gotten worse until I tool the whole thing offline. Now it will crash in whatever state. If I am in Ravensthorpe, it will just freeze, and I must kill the PC manually. If anywhere outside Ravensthorpe if will CTD – Recently played through Ireland and France DLC – had 36 CTD in short amount of time.
